Step 2: Print out and complete our Model Release Form.

Step 3: Send us your completed Model Release Form and a photocopy of your valid ID. This is used for age verification and is required by law. We must recieve your age verification and Model Release Form before your cam goes live or it will be disabled until we recieve it.
